Temporary Guardianship Agreement with Detailed Authorization
Regarding the Acquiring of Medical Care for Minor Children
(Including Consent of Temporary Guardians) TEXAS

Will this form be sufficient to establish temporary Guardianship. My wife was directed to do this because she has her first son, and together we have 2. Total of 4 dependants. Though she was told she could carry me and our two children then later regain Gaurdianship after her training due to the dependant eligibilty?

After Notorizing the Form does she need to file it with the courts? Or is Notorizing it the same?

Will the Navy Need to see the Original Forms?

You will need to direct those questions to the Recruiter-In-Charge of the recruiter station that you are using.

The wording of the agreement will need to be reviewed .... they can also guide you on the notorization requirement.

My wife comfirmed that is the doc and notorizing with our bank or a commission notorizor is suffice. She has been shceduled for her MEPS this Friday all day, they will put her up in a hotel Thursday @ 13:00. I hope the best for her, yesterday I nearly broke my voice buckled as we were serveing dinner, man this is a big one! But I gotta be stong. Please Pray for us..
